Case Manager – New York Team
Golden Care Therapy
Golden Care Therapy is seeking a driven, compassionate, and highly organized Case Manager to join our growing New York team. This role is ideal for someone who is passionate about supporting families and staff, thrives in a fast-paced environment, and excels at communication, problem solving, and relationship-building.
As a Case Manager, you will serve as a critical connection between our families, clinical teams, and administrative staff—ensuring high-quality service delivery, clear communication, and smooth day-to-day operations.
Key Responsibilities
Serve as the primary liaison between the office, families, and clinical staff
Coordinate, monitor, and track client reassessments and authorizations
Review and monitor weekly billing trends to ensure accuracy and follow-up
Conduct monthly care calls with families to provide support, address concerns, and maintain strong relationships
Assist with schedule changes, staffing needs, and case-related concerns
Proactively identify issues and collaborate with internal teams to develop effective solutions
Track, organize, and maintain all required client documentation in compliance with company and regulatory standards
Skills & Qualifications
To succeed in this role, you should bring both heart and hustle . The ideal candidate will have:
Strong administrative experience with proficiency in Excel and Outlook
Excellent phone etiquette, call management skills, and confidence speaking with families and staff
Outstanding problem-solving abilities with a proactive, solution-oriented mindset
Exceptional time management and organizational skills
Knowledge of Applied Behavior Analysis (ABA) and an understanding of behavioral health concepts and interventions
Strong communication and interpersonal skills, particularly when working with individuals with Autism Spectrum Disorder (ASD) and their families
Ability to remain calm, professional, and empathetic in high-pressure or sensitive situations
Bilingual in Spanish is a plus
