Requirements:
Experience: 2 years Case Manager experience plus Oncology & PEDS experience
\tCertification: PALS or PEARS
\tCharting System: Not specified
\tPatient Age Group: Neonates, Infants, Pediatrics, Adolescents
\tFloat: Yes – covers other colleagues
\tOn-Call: None
\tDaily Census: 20–22
\tChemotherapy: No administration; must know chemo interactions & Roadmaps
\tSkills Required:
\tRoadmap development (custom per patient)
\t\tCare Coordination & Transition Planning
\t\tFollow-up appointments, referrals, authorizations
\t\tOrdering/follow-up of labs, scans, DME
\t\tNeeds assessment
\t\tNeuro-Oncology experience
\t\tPreferred:
