Location: Priority Xtracare (On-site / Hybrid)

Position Summary:

The Triage Nurse / Medical Assistant plays a key role in ensuring high-quality patient care coordination through clinical triage, referral management, and accurate documentation. This position supports the medical team by assessing referrals, managing patient case files, coordinating with Home Health agencies, and facilitating ongoing patient care plans. The role requires strong analytical skills, clinical judgment, communication abilities, and attention to detail.

Scope of Work & Key Responsibilities:

Clinical Triage & Patient Case Management:

  • Review, triage, and manage patient case files and referrals in a timely and organized manner.
  • Assess accuracy and completeness of all incoming referrals; escalate discrepancies or missing documentation.
  • Attach detailed clinical analyses and notes to the patient's electronic health record (EHR) using encounter documentation.

Patient Care Coordination:

  • Collaborate with the care team to facilitate appropriate care plans based on patient needs, including assessments, diagnoses, and progress monitoring.
  • Coordinate with referring providers and Home Health Agencies (HHAs) to ensure care goals are met and services are rendered.
  • Conduct patient handoffs by obtaining updates from HHAs, including medical records, qualified/unqualified evaluations, skilled nursing (SN), physical therapy (PT), occupational therapy (OT), and the duration of care provided.

Referral Oversight:

  • Review and track referral intake to confirm that all necessary orders have been received, fulfilled, and logged accurately.
  • Ensure efficient and accurate data exchange between providers and HHAs to maintain continuity of care.
  • Follow up with providers on order statuses, patient contact efforts, and next scheduled appointments.

Communication & Reporting:

  • Maintain professional and timely communication with providers, HH partners, and internal care teams.
  • Participate in clinical report exchanges with HH nurses to evaluate patient progress, identify gaps in care, and support ongoing treatment plans.

Compliance & Ethics:

  • Adhere to all HIPAA regulations and maintain the confidentiality of patient information.

  • Abide by the organizations Code of Ethics and internal policies outlined in employee handbooks and protocols.

Qualifications:

  • Licensed Practical Nurse (LPN), Registered Nurse (RN), or Certified Medical Assistant (CMA) preferred.
  • Minimum 12 years of clinical experience in triage, referral coordination, or a medical assistant role.
  • Strong working knowledge of EHR systems, medical terminology, and care coordination processes.
  • Excellent communication, analytical, and organizational skills.
  • Ability to multitask in a fast-paced environment while maintaining a high level of accuracy and professionalism.

Working Conditions:

  • Full-time schedule: hours may vary based on patient needs and provider availability.
  • Position may involve prolonged periods of sitting, phone communication, and EHR system use.
  • Must maintain compliance with HIPAA and clinical quality standards at all times.
